Hints i.e. what aided me— While still hospitalized my family was able to get medical power of attorney and file for disability. Being able to get things started for me early enough so they would be ready when I was discharged was a tremendous help to me.

Out patient rehabilitation—Life-skills are taught in a variety of community and facility-based settings. They are necessary for successful community integration and functional independence. Life-skills are adaptive behaviors that are required to deal effectively with the demands of daily living. (Your physician can refer you to a center in your area if he thinks this options suits your needs)
Assisted Living - independence is offered as much as you want knowing that personal care and support services are available needed. These communities provide residents with assistance with basic activities of daily living i.e bathing, dressing, and more. Depending on the state they may also allow assisted living to dispense medications.
Independent Living - being rehabilitated to live like everyone else. To have opportunities to make decisions that affect your life and be able to decide upon your own activities.
